To register for the team, please review and fill out each of the forms in the Registration Packet.  These include:

• Family Membership form (with annual membership fee) - per family
• Coaching Contract (with first month's coaching fees) - per family
• Pacific Swim form (with $60 annual membership fee) -per child
• Medical Information form - per child
